Every person born in the United States is issued a unique social security number. This number is printed on a card and sent to the recipient upon request. Because applications are usually made when a baby is born, it is likely that the card can be lost before the baby reaches adulthood. If you live in New Jersey, and you lose your Social Security card, you can get a new one.

  1. Visit the Social Security Administration 's local office search page (see Resources) to find the nearest New Jersey Social Security.

  2. Download and print the form SS-5: Application of social security card (see Resources). Fill out this form.

  3. Browse your personal documents and to find a document that proves your identity, and another to prove your U.S. citizenship. Good materials to use include driver's license 's, passports, visas and birth certificates.

  4. Take your paper Form SS-5, which prove your identity and your document proving U.S. citizenship at the nearest New Jersey Social Security Bureau. Deliver the documents to the clerk.

  5. Wait for the Social Security Office to review your documents and send them back to you. Shortly after that you should also get your new Social Security card in the mail. Your Social Security number will remain the same. Only the card will be new.
